About the Author
Trevor Kletz, OBE, D.Sc., F.Eng. (1922-2013), was a process safety consultant, and published more than a hundred papers and nine books on loss prevention and process safety, including most recently Lessons From Disaster: How Organizations Have No Memory and Accidents Recur and Computer Control and Human Error. He worked thirty-eight years with Imperial Chemical Industries Ltd., where he served as a production manager and safety adviser in the petrochemical division, also holding membership in the Department of Chemical Engineering at Loughborough University, Leicestershire, England. He most recently served as senior visiting research fellow at Loughborough University, and adjunct professor at the Mary Kay O’Connor Process Safety Center, Texas A&M University.

awesome information about what went wrong so that it doesn't happen again.
I had read the other two recent books by Kletz, 'Still going wrong' and 'Learning from accidents'. The style is easy to read and the stories are really fascinating if you are into safety and industrial chem. The only negative really is that the three books contain similar content to each other in that they overlap with the same stories for say 50% of the items. Regardless, this particular volume WWW 5th Ed is an extremely good reference tool for safety and process professionals.
Strongly recommended for chemical engineers. I can't flip to a page of this book without finding something new and, more often than not, relevant to what I'm working on.

A must read for everyone who is involved in or has any relation to process plant design, operations, maintenance or management. Provides a deep understanding of the industry and its risks. The fifth edition has the second book Still Going Wrong from 2003 as part B. Simply great.

The Instrumentation and Controls sections are slightly out dated. Everything else is outstanding. Many great examples and corrective actions are covered.
